Diabetes Educator (In-Patient)

Position Summary

Provides state of the art CDCES diabetes education assessment, diabetes self-management education (DSME) to patients and their families/care-partners, helping to optimize their diabetes management. Will assists with the responsibility for the assessment, planning, implementation and evaluation of the patient centered diabetes education program at Stony Brook Medicine. Collaborates with other educations and members of medical team in identifying the needs, developing and providing clinical plans.

Duties of a Diabetes Educator (In-patient) may include the following but are not limited to:



  • Provided 1:1 DSMES in inpatient setting.
  • Responsible for communication with referring primary medical team.
  • Support instruction in diabetes education for the nursing orientation and critical care.
  • Active member of the Diabetes Advisory Committee.
  • Assists in the updating of materials needed to keep Diabetes program within The Joint Commission advanced certification in inpatient diabetes care.
  • Assumes responsibility and accountability for planning, teaching and evaluating the delivery of clinical nursing- Diabetes Self Care Management Education (DSME/S) for adult patients with diabetes.
  • Maintain and document in patient records per standard of care and to meet requirements for certification.
  • Ongoing review and assessment of program resources.
  • Responsible for upkeep and ordering of DSME education supplies.
  • Maintains professional registration and certification and state licensure. Attend continuing education programs, conferences to maintain CDCES and RN credentials.
  • Evaluate and assess patient's management and education needs, self-care behaviors, and obstacles to successful implementation of therapy in a variety of settings.
  • Provide formal training on diabetes management and risk factor reduction.
  • Documents and processes medical records, and statistical records according to Stony Brook Medicine Policy and Procedures.
  • Identify discharge needs at the point of initial patient contact and assist in transition of care planning.
  • Demonstrates skills in collaboration, teamwork and problem solving to achieve goals.
  • Knowledge of basic software packages and hospital information systems (EMR).



Qualifications

Required:



  • Registered Nurse with current NYS license.
  • Obtain certified Diabetes Care and Education Specialist (CDCES) within 1 year of hire.



Preferred:



  • CDCES with experience teaching comprehensive diabetes self-management education.
  • 2-5 years' experience specializing in Diabetes program management and development, research experience, and quality assurance experience.
  • Knowledge of current clinical guidelines and standards from the American Diabetes Association, American Association of Clinical Endocrinologists and American Association of Diabetes Educators.

  • Complete electronic reference check with a minimum of three (3) professional references.
  • Successfully complete a 4-panel drug screen*
  • Meet Regulatory Requirements for pre-employment screenings.
  • Provide a copy of any required New York State license(s)/certificate(s).

Anticipated Pay Range:

The starting salary range (or hiring range) for this position has been established as - $90000 - $105000.

The above salary range (or hiring range) represents SBUH's good faith and reasonable estimate of the range of possible compensation at the time of posting.

In addition, all UUP positions have an additional $3,400 for location,
