(NP) Nurse Practitioner - Hepatology - 141483

Under general supervision, the Hepatology Nurse Practitioner (NP) - Research provides comprehensive, independent care to patients with chronic, advanced, and medically complex liver disease within a fast-paced high-volume ambulatory specialty practice. The NP provides independent clinical assessment and management of new and established hepatology patients, including patients with metabolic dysfunction-associated steatotic liver disease (MASLD), viral hepatitis, cirrhosis and its complications, portal hypertension, hepatocellular carcinoma (HCC), and liver transplant-related conditions. Responsibilities include conducting diagnostic evaluations, developing and modifying treatment plans, prescribing and managing therapies, monitoring disease progression, coordinating specialty and procedural care, and recognizing situations requiring physician consultation or escalation.

This is an experienced APP role requiring a high degree of clinical autonomy, advanced clinical judgment, and the ability to independently manage medically complex patients across the continuum of hepatology care.

The NP performs comprehensive histories and physical examinations; orders, reviews, and interprets diagnostic testing; develops and implements individualized treatment plans; prescribes and manages therapies; provides patient education and counseling; and coordinates diagnostic, procedural, specialty, and longitudinal follow-up.The NP will independently manage a patient panel in the ambulatory setting and collaborate closely with hepatologists, APPs, nurses, pharmacists, dietitians, transplant teams, research teams, and other multidisciplinary providers to ensure timely, coordinated, and comprehensive care.

The NP provides longitudinal management for patients with chronic and advanced liver disease, including high-complexity medical decision-making, diagnostic review, medication management, patient counseling, and multidisciplinary care coordination. The NP reviews hepatology-specific diagnostics, including liver imaging, elastography/FibroScan, MRI/MRE, laboratory studies, pathology, and liver biopsy results to guide evidence-based treatment and ongoing disease management.

High-Volume Ambulatory Practice and Patient Access

The NP independently manages a high-volume ambulatory practice that includes new consultations, follow-up visits, urgent referrals, telehealth encounters, and ongoing disease management. The role supports division-wide patient access initiatives by helping maintain timely appointments, continuity of care, quality outcomes, documentation standards, and efficient clinic throughput.

Additional clinical responsibilities include:

* Management of urgent hepatology referrals and clinical concerns
* Diagnostic testing and results
* Medication management
* Epic In Basket and MyChart communications
* Patient education and counseling
* Telehealth visits, including video and telephone encounters
* Timely and accurate clinical documentation

The NP is expected to exercise independent clinical judgment within an interdisciplinary and multidisciplinary approach to patient care. Other duties may be assigned based on clinical, operational, patient access, and programmatic needs.

MASLD Research Center and Clinical Research

The NP will participate in hepatology-related clinical research and protocol-driven care through the MASLD Research Center, including supporting patients enrolled in clinical trials, assisting with protocol-specific clinical management, coordinating with investigators and research coordinators, and helping ensure continuity between research activities and clinical care.The NP may participate in protocol-driven assessments, review clinical and research-related diagnostic information, provide ongoing disease management, and collaborate with multidisciplinary research teams.

The position supports the broader clinical, operational, access, and research missions of the Hepatology program.

The Nurse Practitioner will assess and provide nursing actions as appropriate for patient care populations based on competence, professional expertise, and knowledge. The nurse is expected to practice consistent with ANA Standards of Practice and Professional Performance. Independent judgment is expected to be exercised within the context of interdisciplinary and multidisciplinary approaches to patient care. Under general supervision, incumbents perform operational level duties as described in the Series Concept. The major emphasis at this level is ongoing direct patient care delivery. Assignments at this level primarily encompass management of patients who present chronic and/or episodic health problems. Assignments at this level may include responsibility for occasional community teaching programs, participation in program planning and development of patient education information. Job standards are to be performed at the PRACTITIONER level.

• Graduate from an accredited nurse practitioner program.

• Current California Registered Nurse (RN) license, California Nurse Practitioner (NP) license, and Furnishing License Certification.

• Current Nurse Practitioner Specialty Certification in an aligned specialty with the practice area from one of the following Nursing Boards: AANP, ANCC, NCC, PNCB, AACN.

• BLS/CPR certification at time of hire with commitment to obtain BART within 6 months.

• Six (6) months or more of experience as a licensed Nurse Practitioner.

• Direct hepatology clinical experience is required, with demonstrated ability to independently evaluate and manage patients with chronic and advanced liver disease, including cirrhosis and its complications. Experience caring for patients with liver disease, specifically NAFLD, is required. Five (5) to seven (7) years of related hepatology experience is preferred.

• Demonstrated experience performing hepatology-specific diagnostic tests, exams, and procedures, including FibroScan/VCTE, elastography, HVPG, and liver biopsies.

• Demonstrated ability to perform comprehensive physical examinations and obtaining precise medical history and pertinent data.

• Demonstrated knowledge of clinical laboratory values and skill in identifying deviations.

• Demonstrated ability to assess staff education needs, and design appropriate in-service education programs. Demonstrated ability to develop educational materials and provide staff and patient education.

• Strong computer skills, including experience using Electronic Medical Records, EPIC proficiency.

• Excellent written and verbal communication skills, with the ability to work effectively in a multidisciplinary team environment and provide care to diverse adult patient populations.

• Proficiency in clinical documentation and compliance with applicable standards of care, policies, and procedures.

• Experience in clinical research, including protocol adherence, patient screening and recruitment, FDA regulations, informed consent requirements, and protection of human subjects.

• Experience working in high-volume ambulatory practice, independently managing complex patient populations with multiple comorbidities.

• Experience providing care through telehealth services including patient assessment, triage, and counseling.

• Demonstrated experience with hepatology medication management, including the development, implementation, and monitoring of complex treatment plans.

• Record of submitting NAFLD/MALSD related scholarly publications.

• Experience independently managing patients with MASLD, viral hepatitis, cirrhosis, HCC, portal hypertension, and liver transplant-related conditions.

• Experience working within an academic medical center, tertiary referral center, or multidisciplinary specialty practice environment.

• Demonstrated experience with patient triage, care coordination, population health initiatives, and complex care management.

• Experience participating in quality improvement, operational workflow development, patient access initiatives, or program development.

• Must be able to work various hours and locations based on business needs.
