Your Role The Community Health Advocate team is responsible for improving health outcomes and reducing barriers to care for Blue Shield Promise Health Plan members through field-based outreach, care navigation, community engagement, and support services that address social drivers of health. The Manager, Community Health Advocate (CHA) will report to the Sr. Director, Community Programs. In this role, you will lead a team of field-based Community Health Advocates who conduct home visits, accompany members to medical appointments, perform social needs assessments, connect members to community resources, and provide hands-on support to improve access to care. You will be responsible for the day-to-day leadership, performance management, coaching, and development of the CHA team while ensuring high-quality member engagement and service delivery. Hybrid This role requires employees to be in-office based on our hybrid workplace model, balancing purposeful in-person collaboration with flexibility. For most teams, this means coming into the office two days each week. Employees living more than 50 miles from an office location will work with their manager to determine in-office time based on business need. | |
