SALARY RANGE $30.32 - $42.11WORK ASSIGNMENT DETAILS Waukesha County Department of Health & Human Services is recruiting for a talented professional to join its team as a Targeted Case Manager/Community Recovery Services (CRS) Monitor (Senior Mental Health Counselor classification). The current opening is in the Outpatient Behavioral Health Division and is responsible for providing case management services to adults with serious and persistent mental illness who reside in licensed group home settings. This position supports adults enrolled in Waukesha County's Targeted Case Management and CRS programs. Responsibilities include establishing, coordinating, and monitoring both in-county and out-of-county group home placements to ensure services continue to meet each individual's needs. The position also requires travel throughout Southeastern Wisconsin approximately 1-2 days per week to conduct on-site visits at residential providers; this includes monitoring placement quality, assessing client progress, and collaborating with providers and treatment teams. This position works as part of an interdisciplinary team that includes clinical supervisors, psychiatric prescribers, nursing staff, and licensed group home providers. The selected candidate will have a strong understanding of mental health disorders and the behavioral health continuum of care, along with the ability to build collaborative relationships with clients, providers, and community partners. Knowledge and application of exceptional customer service, psychosocial rehabilitation, person-centered planning, and effective case management practices are essential. Previous experience providing case management services to adults with serious and persistent mental illness and co-occurring substance use disorders is preferred.

Bachelor's degree from a recognized college or university with at least four (4) courses in sociology, social work, or psychology.
