Elderwood of Scallop Shell at Wakefield is seeking a compassionate and dedicated Social Worker to join our interdisciplinary team and support the emotional, psychosocial, and transitional needs of our residents and their families. Position OverviewAs a Social Worker, you will play a vital role in enhancing the quality of life for residents by identifying psychosocial, emotional, and social service needs and connecting residents with the resources and support necessary to meet those needs. You will work closely with residents, families, nursing staff, and community partners to promote dignity, independence, and overall well-being while helping residents successfully navigate transitions of care. In This Role, You Will:
Manage a social work caseload for residents on assigned nursing units
