Job Summary

This position organizes, coordinates, and delivers comprehensive physical therapy services to patients in the home care setting (home health and hospice for pediatrics and adults), ensuring care is effective, patient-centered, and aligned with best practices. Serves as a clinical specialist, maintaining advanced, professionally recognized expertise in the field, and remaining current with developments in physical therapy and home-based care. Applies this knowledge to enhance patient outcomes, support interdisciplinary collaboration, and share expertise with the broader healthcare community.

The primary purpose of physical therapy is to provide a full range of rehabilitation services designed to restore patients to their optimal functional capacity.

Minimum Qualifications - For full consideration, applicants are encouraged to upload license and/or certification if required of the position

  • California Licensed Physical Therapist (PHYSTHER)
  • Current California Physical Therapy License. Must supply all appropriate information to maintain hospital credentialing in a timely manner.
  • American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) or UC Davis Health CPR
  • Required to hold a valid California driver's license, have a driving record that is in accordance with local policies/procedures, and/or enroll in the California Employer Pull Notice Program
  • Baccalaureate degree in Physical Therapy
  • Must have three or more (3+) years of experience in the practice of physical therapy
  • Demonstrated advanced operational/specialist skills and knowledge in physical therapy as identified by Physical Therapy Practice Act and licensure by State of California.
  • Ability to see and hear well enough to observe patient physical reactions and assess accordingly.
  • Demonstrates excellent clinical judgment skills, accountability, and professionalism in patient-centered care.
  • Commitment to continual excellence and performance improvement in patient-centered care.
  • Adapts to change in a positive, professional, and productive manner.
  • Excellent verbal and written communication skills to effectively communicate with a diverse patient and staff population in a clear, concise, and professional manner.

  • Ability to provide and accept guidance and criticism.

  • Ability to assess, plan, implement, and evaluate patient care, taking into consideration protective interventions, development (behavioral and physical), and physiological characteristics specific to the patients.

  • Master's Degree or Doctorate Degree in Physical Therapy

Department Specific Job Scope

Home Care Services is made up of three primary programs: Hospice (inpatient and home care), Home Health, and Home-Based Palliative Care. This position is part of the Home Health Program.

The incumbent primarily works in the field as this is an in-home care position. Occasionally, the incumbent works in the HCS departmental office located in Rancho Cordova for meetings, training, and additional, administrative tasks.

Work Environment

UC Davis is a smoke and tobacco free campus effective January 1, 2014. Work environment may include clients' homes, hospitals, care facilities, and clinics.
