Individualized Home Supports Case Manager

Position Summary

Individualized Home Supports (IHS) empower people with disabilities to live more independently. In this role, you'll provide one-to-one, person-centered support-at home, in the community, or remotely-to help individuals build daily living skills and increase independence through customized, skill-focused services.

Essential Functions:

• Support daily decision-making and help individuals make informed choices in all areas of life.
• Assist with daily living tasks including personal care, meals, transportation, housekeeping, and laundry.
• Maintain a safe, comfortable home environment that supports personal goals.
• Teach and build skills in community participation, health and wellness, safety, communication, social interaction, and household management.
• Promote problem-solving and self-advocacy through education and real-life opportunities.
• Administer and document medications and provide required health-related supports.
• Encourage positive behavior and follow individualized behavior support plans.
• Respond to crises using prevention, intervention, and safety techniques; report and document incidents as required.
• Participate in team meetings and complete all required training on schedule.
• Document services and progress accurately, including daily notes and billing times.
• Manage a caseload of 20+ individuals with professionalism and consistency.

Required Education, Experience, and other Qualifications

1. Technology skills: Proficient with Microsoft Office software (Outlook, Word, etc.)
2. Case management - ability learn software system and become proficient
3. Current, valid MN driver's license
4. A driving record that demonstrates safe, legal driving habits
5. Compliance with Rule 11 and MVR background check required?

Preferred Education, Experience, and other Qualifications

• Previous experience practicing person-centered techniques with people with disabilities
• Degree in human service field
• Certification in medication management

Working Conditions and Environment:

Hours of Work

Hours of work are typically daytime, Monday through Friday (No evenings, weekends, or major holidays)

Travel

Regularly transport people in the community using personal vehicles. Rise van/buses may be available upon timely request.

Work Environment

Services are provided in the clients' home, (independent living arrangements, family home) or in the community. This position may also follow a hybrid work schedule, allowing for remote work when appropriate. In addition, staff may utilize accessible internal workstations during times when they are not providing services in the client's home.

Physical Demands

The physical demands described here are representative of those that must be met by a team member to successfully perform the essential functions of the job.

Direct Support Care could include lifting and transferring adults to/from wheelchairs with varying weights.

The team member is required to twist, stoop, bend, reach, pull, push, stand, sit and walk for up to eight hours per day
