Overview

NaphCare is hiring a strong Jail Based Behavioral Health Services (JBBS) Discharge Planner to join our team at the Mesa County Jail located in Grand Junction, CO.

Pay rates range from $23.00-$26.00 per hour based on shift, site needs, hire status, and experience.

This position is part of the state supported Medication Assisted Treatment (MAT) and Substance Use Disorder (SUD) JBBS program that provides targeted mental health services for adults patients detained in jail. The position works under the direct supervision of the JBBS Program Director who provides training, support, and professional development for this role.



Responsibilities

The JBBS Discharge Planner is responsible for conducting intake and outcomes assessments as well as serving as a liaison to all case management and discharge planning for this specialized sub-population within the facility. Duties and responsibilities may include and are not limited to:

    Conduct demographic and basic psychosocial history inventories with JBBS participants
  • Manage (enter/maintain) JBBS data in the electronic health record and respective databases
  • Develop, maintain, and distribute community resource lists and access for JBBS participants
  • Work closely with the JBBS team, mental health, and medical to coordinate JBBS patient care
  • Work closely with the Mesa County Detention's Center Transitions' Team to collaborate regarding identifying discharge needs and discharge planning for the JBBS population
  • Provide evidenced-based psychoeducation to JBBS MAT patients individually, as needed
  • Attend JBBS and NaphCare trainings and staff meetings
  • Coordinate care and build relationships with community agencies and organizations
  • Relay critical information to the next level of management for patient care needs
  • Work collaboratively with custody partners and NaphCare team members
  • Integrity, professionalism, and demonstrated ability to adhere to a Code of Conduct and comply with facility correctional healthcare policies, procedures, and legal requirements


Qualifications

Experience / Requirements / Qualifications

  • High school diploma or equivalent; an Associates or Bachelor's Degree in a human service (e.g., psychology, social services), medical, or criminal justice field is strongly preferred
  • Knowledge and/or experience in the mental health, substance use disorder, and/or Medication Assisted Treatment (MAT) fields strongly preferred
  • Correctional healthcare experience preferred
  • Proficiency in data management applications (e.g., Excel) and/or electronic health records
  • Excellent communication skills for interacting with patients, staff, and custody
  • Ability to work conscientiously and independently and as part of a team
  • Valid CPR card required (may be obtained at any point prior to hire)
  • Must pass the facility's criminal background check and drug screening

Physical Capabilities:

Must have the ability to stand for long periods, lift and move patients (up to 50 pounds), bend, squat, reach, push, pull, walk significant distances, and perform physically demanding tasks like CPR, all while maintaining good balance and endurance throughout a shift.
