Social Worker

Capitol View Transitional Care Center is a state-of-the-art true transitional care unit (TCU) located in Regions Hospital (we are part of the HealthPartners organization as a separate nonprofit organization.) We provide quality care to patients from 18+ years of age who are in need of a short-term stay. Our average length of stay is 17 days. As a Social Worker, you will be responsible for the following job duties and responsibilities:

* Preforming psychosocial and mood assessments.

* Following up on needs determined in assessment and care planning for dc, mood, behaviors, and psychosocial issues.

* Preforming ongoing care, discharge, and transition planning and coordination.

* Arranging for services, referrals, and providing resources to patients. Referring for home and outpatient follow up care, and placement and transfers for assisted living.

* Tracking and inputting insurance case management information, while communicating updates and providing information related to insurance carriers and providing notices of noncoverage and patient appeal rights and processes.

* Participating in meetings including IDT meetings and rounds.

FTE: .5 (20 hours per week)

Work Schedule: Monday-Friday with flexibility

Required Qualifications:

* Bachelor's degree in Social Worker

* LSW or higher

* One (1) year of social work experience
