RN Case Manager Hospice

Full time, 8:00 a.m. to 4:30 p.m., variable days and on call weekends

Hybrid: Home and Facility Visits, travel to patients' homes, nursing facilities, and hospitals to provide direct care in Niles/Buchanan and surrounding areas.

Corewell Health Hospice specializes in providing end of life care and symptom management to individuals with a life expectancy of six month or less, serving individuals in their home, nursing facilities, and in the hospital. The health system comprises threehospitals, an outpatient surgery center, a regional cancer center, rehabilitation centers, two long-term care residences, home care and hospice services, and 34 affiliate physician practice locations. We provide both inpatient and outpatientmedical servicesthroughout Southwest Michigan.

Under the direct supervision of the Nursing Manager, the Hospice Nurse Case Manager is responsible for coordinating and delivering compassionate end-of-life care to patients and their families. This role involves assessing patient needs, developing individualized care plans, and collaborating with an interdisciplinary team to ensure the highest quality of hospice care.

Required

• Associate's degree in nursing or equivalent
• 2 year of relevant experience preferably in Hospice
• Michigan Registered Nurse (RN) license upon hire
• State of Michigan Driver's license upon hire

Preferred

• Bachelor's degree
• 1 year of relevant experience in Hospice
