The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Job Overview:

Licensed Social Workers are responsible for providing clinically effective treatment, case management, and care coordination. They shall provide all patients with weekly individual sessions, a minimum of bi-weekly family therapy sessions, and crisis intervention as needed.

Licensed Social Workers are responsible for conducting psychosocial assessments, risk assessments, developing individualized treatment plans, and engaging the children and families as an active voice in their own care. They shall cohesively coordinate treatment with the multidisciplinary team, consisting of the Psychiatrist/APN, RN, Creative Arts Therapists, Managed Care Specialist, and Family Specialists/Program Assistants to deliver the highest quality of care and promote the children's health, safety, and well-being.

Qualifications:

Required:

  • Master's degree in Social Work

Preferred:

  • One year previous experience in a healthcare setting

Certifications and Licenses Required:

  • NJ Licensed Social Worker

Scheduling Requirements:

  • Shift working, 4 days per week 8:00-4:30pm or 8:30-5:00pm, and 1 evening per week, i.e., 11:30-7:30pm or 12:00-8:00pm
  • Full-Time (FT)

Essential Functions:

  • Completes Intake psychosocial assessments, including risk assessments, in compliance with all required medical record standards for each child and family
  • Assesses children s needs through clinical interview, chart/history review, and through use of standardized instruments
  • Interprets nature of illness and prescribed course of treatment to children and families; completes treatment plans following the S, M, A, R, T model; conducts individual, group, and family therapy; and evaluates and documents patient outcomes according to policy and procedure
  • Actively engages families in their child s treatment and ensures the child s and family s voices are captured and documented in their treatment planning aimed towards successful discharge
  • Communicates all relevant child/family issues that arise during treatment to other staff members
  • Participates in all treatment planning conferences with multidisciplinary team and families
  • Analyzes data from outcome measurement tools to formulate comprehensive treatment plans and modifies treatment objectives, approaches, and techniques accordingly
  • Maintains up-to-date and timely clinical and case management documentation in the EMR of patient information according to established standards and regulations
  • Expedites, coordinates, and communicates child s admission and discharge with family and all staff, including Intake Coordinator, Managed Care Specialist, Clinical Coordinator, RN, Family Specialists, and Administrative Assistant
  • Collaborates routinely with other hospital/system programs, referral sources, and community agencies with which patients and families are involved, and as part of aftercare planning
  • Provides crisis management support to staff and meets with children individually, as needed, in addition to weekly sessions
  • Actively participates in clinical supervision in accordance with NJ Board of Social Work regulations
  • Supervises interns as per university field instruction requirements when requested
  • Routinely attends staff meetings/huddles and brings agenda items for programmatic improvements
  • Will complete all mandatory hospital/system employee trainings each calendar yeah
  • Performs other duties as required
