Description

The FLiP Patient Navigator Program and the FLiPRx Produce Prescription Program are grant-funded projects to build clinical-community collaborations to support equitable access to nutrition and activity resources for families experiencing food insecurity and/or chronic diet-related health conditions within DC. Our program trains medical student volunteers to serve as patient navigators; when clinicians receive a positive food insecurity screen, or have a family experiencing a diet related chronic health condition that would benefit from more nutrition access and education, they can refer to FLiP. Our student navigators participate in a 2-part training curriculum, as well as participate in quarterly meetings that expose them to a variety of topics relevant to health inequities and nutrition access. These students then connect families with community resources, screen for eligibility for federal nutrition programs, and screen for eligibility for our 6 months produce prescription program. We have historically focused on serving families within Wards 7 and 8 but are expanding our referral base to other pediatric medical homes throughout DC.

With this expansion we are looking for an individual that is comfortable working remotely via the phone with community members to provide direct resource navigation, as well as providing administrative support for the broader patient navigator program.

Position Summary

Assist staff in the daily execution of program tasks and projects, providing administrative support to Program Manager and Program Coordinator. Responsible for maintaining office files, completing purchase and reimbursement requests, as well as managing financial records. Assist with fundraising by researching funding prospects, drafting communication with prospective donors, and managing logistics for fundraising events. Assist with special projects related to program goals and mission.

Responsibilities:

  • Monitor RedCap (HIPPA compliant data capture platform) referrals and assign weekly referrals to students

  • Monitor Mosio (HIPPA compliant texting platform) to ensure text replies are completed and all resource documents are sent

  • Manage referral database to ensure families are called within 2 weeks of referral - often will require making resource navigation calls estimated to take ~8hrs/week

  • Work with medical student groups to ensure community resource guide is up to date -Supporting role in bi-annual training session and new navigator onboarding

  • Send monthly referral reports to clinic partner sites

  • Work with faculty leadership, clinical partner sites, and produce prescription partners to ensure efficient workflow

Qualifications

Minimum Education
High School Diploma or GED (Required)

Minimum Work Experience
1 year performing administrative tasks (Required)

Required Skills/Knowledge
Proficient in all Microsoft Office applications, including Outlook, Excel, and Access.
Ability to produce quality reports under tight deadlines.
Ability to maintain composure and professionalism in a busy clinic environment.
Web design and desktop publishing skills a plus.

Experience with RedCap (HIPPA compliant data capture platform) is a plus.
