Job Summary


The Nurse Coordinator assists with navigating all aspects of the healthcare system. Within the Care Transitions Team, the Relay Center and Discharge Coordinator Programs are charged with reporting Laboratory, Microbiology, and Radiology test results to patients or internal/external providers who ordered tests that were performed at UCSF. This could include test results from the Emergency Department, Inpatient or the Ambulatory care settings. The Nurse Coordinator collaborates with various internal and external partners to ensure that patient results are handled appropriately and according to standard procedure. This position may involve coordinating care for patients without primary care providers, assisting with referrals and scheduling follow-up testing, and facilitating connections to community resources. The Nurse Coordinator is responsible for triaging patient symptoms and handling clinical escalations from the team's Health Care Navigator(s). The Care Transitions team works to meet patient expectations, achieve quality outcomes, and build a patient-focused culture. In addition, all Care Transitions staff work with leadership to develop workflows and standard operating procedures, assist with quality improvement projects, and track key performance metrics and outcomes.

This Nurse Coordinator role is part of a dynamic care team within the Office of Population Health (OPH) Clinical Programs. This position will report to the Manager, Care Transitions Programs. The Nurse Coordinator is a hybrid position with requirement for travel to all UCSF Health locations which includes the Brisbane Office at 8000 Marina Blvd in Brisbane. The incumbent will be required to reside in the state of California within travel distance to San Francisco. The incumbent should be able to provide a safe and private home office environment for conducting patient outreach and UCSF business.

The final salary and offer components are subject to additional approvals based on UC policy.

Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.

The salary range for this position is $121,900 - $277,100 (Annual Rate).

Department Description

The Office of Population Health (OPH) leads UCSF's development of innovative care models to improve the value and quality of care and improve health outcomes for identified populations and facilitates implementation of these care models across the health system. OPH is responsible for fulfilling UCSF Health's accountable care contracts and fostering care partnerships across a clinical network. This department is comprised of four main teams: (1) Clinical Programs, (2) Population Health Quality and Performance Improvement, 3) Ambulatory Risk Capture/Medicare Advantage Operations, and 4) Population Health Administration.

Required Qualifications


  • Minimum of three (3) years of recent experience as a registered nurse caring for patients in an acute care setting
  • BSN - graduate from an accredited school of nursing
  • Active Registered Nurse licensure in the State of California
  • Active American Heart Association BLS Certification
  • Strong knowledge of medical terminology
  • Proficiency with EPIC APeX applications
  • Excellent written and verbal communication skills
  • Proficiency with Windows-based software including Microsoft Word, Excel, Outlook, Teams
  • Ability to work independently in a fast-paced, demanding environment with minimal supervision
  • Ability to manage and oversee multiple tasks simultaneously, prioritizing and adjusting as needs change

Preferred Qualifications


  • Experience working with test results
  • Knowledge of data collection, compilation, and analytical techniques

License/Certification


  • Active Registered Nurse licensure in the State of California
  • Active American Heart Association BLS Certification
