The Field-Based Community Health Worker (CHW) supports members' health and independence by conducting home visits, assisting with care transitions, and addressing social determinants of health (SDOH). This role acts as a trusted community connector and advocate, helping members navigate complex systems, access essential services, and achieve personal health goals. The CHW collaborates closely with nurses, social workers, and other care team members to reduce barriers and promote equitable, person-centered care.

This is a field-based position requiring regular travel throughout Orange County and surrounding areas (up to 75% local travel).

This position follows a hybrid schedule with three in-office days per week.

Primary Responsibilities:



  • Conduct home and community-based visits to assess member needs, provide coaching, and support the execution of care plans
  • Accompany members to medical appointments and other service-related visits (e.g., housing assessments, benefit office appointments) to provide advocacy and support
  • Identify and address barriers to care such as housing instability, food insecurity, transportation challenges, and social isolation
  • Collaborate with interdisciplinary care teams - including Social Workers, Nurses, EMTs, and Health Advocates-to ensure whole-person care coordination
  • Help members navigate health insurance benefits (Medi-Cal, Medicare), schedule appointments, and access long-term services and supports
  • Provide health education, self-management coaching, and informal counseling, as appropriate to CHW scope
  • Refer members to community-based programs and services, and follow up to ensure effective engagement
  • Attend case conferences and interdisciplinary meetings to escalate and discuss member concerns
  • Maintain timely and accurate documentation of visits, referrals, and outcomes in care management platforms (e.g., Epic, Curo)
  • Adhere to all HIPAA, regulatory, and organizational compliance standards

Required Qualifications:



  • High School Diploma/GED or 5+ years of equivalent experience in outreach, case navigation, or community-based health work
  • Community Health Worker certification; California-recognized programs preferred
  • 1+ years of field-based experience in healthcare, behavioral health, or public service roles
  • Intermediate proficiency in Microsoft Outlook, Word, Excel, and web-based documentation tools
  • Access to a private, HIPAA-compliant home workspace for virtual/telephonic tasks
  • Reside in or near Orange County, CA
  • Valid California driver's license, reliable vehicle, and proof of current insurance
  • Ability to travel locally up to 75%, with frequent same-day visits


Preferred Qualifications:



  • Associate's or Bachelor's degree in public health, human services, social work, or related discipline
  • Experience working with housing programs, IHSS, or social service navigation
  • Knowledge of Epic, Curo, or other care management platforms
  • Familiarity with behavioral health support, Medi-Cal populations, or aging/disabled services
  • Bilingual in Spanish, Vietnamese, Korean, or other locally relevant languages

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $23.41 to $41.83 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
