Summary:

Assist with hiring, orienting, and functional oversight of a team of professional and paraprofessional employees accountable for triaging and prioritizing care to achieve optimal patient outcomes in conjunction with physician orders. Ensures adherence to practice standards, appropriate utilization of services, compliance with federal and state regulations and agency policies in assigning care and triaging patient s needs. Provides oversight for staff scheduling and ensures staff skills match client needs. Ensures appropriate team coordination, geographical sensibility and communication with the physician, interdisciplinary team and managers as appropriate. Utilizes business knowledge and agency objectives to proactively identify staffing requirements and initiate the hiring of proficient caregivers

Responsibilities:

*Assists with hiring, orienting and supervision of staff to include monitoring individual performance, productivity, and quality of services provided. Assist with or performs the annual evaluation of staff performance. Identifies training and development needs.
*Assures accuracy of client and staff schedules, including the amount, frequency, and duration of service that has been identified in the plan of care, physician orders, and/or payer authorization.
*Assures orders and authorizations or re-authorizations for care when needed.
*Assists in the monitoring of daily operations, interprets and educates staff in compliance to, internal policies, and accreditation standards.
*Meets regularly with HH staff to consult regarding caseloads, use of service, and missing documentation of care. Participates in hiring of staff by identifying recruitment needs and unsatisfied customer requests.
*Assures triage of incoming team phone calls and manages those issues that are related to scheduling, customer requests and complaints, caregiver needs. Identifies those calls that require clinical intervention and refers to the appropriate team member. Relays messages to caregiver staff as appropriate.
*Assures that patient needs are identified and met, and that qualified caregivers are assigned consistently.
*Provides counsel in the interpretation of standards and company policies and procedures to all levels of staff as needed.
*Assist with the development and implementation of an outcome oriented patient care plan that conforms to the physician s plan of treatment. Ensures that community and agency resources are utilized to manage the care plan and assure that stated outcomes are achieved. Periodically assist in reassessing patient needs relative to compliance with care plan.
*Supports the interdisciplinary team in all efforts to ensure that all patients receive appropriate care 24 hours a day, 7 days a week which demands close detailed interaction, reporting, and collaboration with after-hours staff.
*Interacts with third party representatives, such as case managers and payers, to obtain needed re-authorizations and negotiate for additional services and supplies for the patient after admission as needed.
*Responsible for maintaining a positive relationship with referral sources and community resources. Contributes to the growth and expansion of Home Health services through delivery of Customer Service, patient advocacy, education of referral sources and physicians, referrals to other products within PHH, and excellent communication with all related parties.
*Provides responsive, customer focused resolution to patient care issues, concerns and conflicts. Informs the appropriate case manager or Home Health Director of significant customer service complaints and issues.
*Participates in Quality Assessment and Improvement activities and assures participation of all appropriate staff. Provides information that enables the collection and root cause analysis of data to identify opportunities for improvement.
*Develops departmental policies, procedures, work flows, goals and objectives; implements PHS policies, procedures and work flows; ensures departmental accountability and customer satisfaction. Communicates pertinent information with employees.
*Maintains professional growth and development through seminars, workshops, current literature and professional affiliations to keep abreast of latest trends.
*Performs other functions as required.

Qualifications:

High school (G.E.D). 3-5 years experience in home care. 1-3 years experience in supervision
