The Guidance Center (TGC) is the Community Mental Health Center and Certified Behavioral Health Clinic serving Leavenworth, Atchison, and Jefferson counties in Kansas. The Guidance Center (TGC) is currently seeking an Adult Community Support Specialist/APP Group Leader. This is a split position providing case management services to SPMI adults, following the strengths model of case management as well as leading psychosocial groups to SPMI or SMI adults who are participating in the Center's Adult Psychosocial Program. Responsibilities Case Management Responsibilities:
- Provide assertive outreach directed toward engaging and maintaining client relationships as well as relationships with caregivers.
- Assist clients in determining strengths and needs, and defining goals, strategies and an action plan which supports community living and participation.
- Participate in the treatment-planning process with clients and other Center providers and collaborate with other providers to ensure appropriateness and timeliness of treatment plan during course of treatment.
- Provide ongoing support and follow-through for client goal attainment in the areas of these life domains: home, financial/insurance, vocational/educational, health, relationships, leisure/recreation, daily living skills.
- Assist clients and/or caregivers to locate and use resources to meet defined goals and needs.
- Assist clients to obtain adequate housing; this could include assistance with obtaining documentation, applying for housing, seeking emergency resources to support housing.
- Transport clients to and from appointments and activities when necessary.
- Assist clients in engaging in Supported Employment services where appropriate.
- Facilitate communication, cooperation and deployment of resources among other service providers and caregivers toward the achievement of defined goals identified on the client's treatment plan.
- Advocate for and with clients as they seek to achieve their goals and overcome barriers.
- Provide response and support to clients and caregivers when emergencies occur.
- Promote and support clients' efforts to manage symptoms.
Adult Psychosocial Responsibilities
- Facilitate groups as assigned and assist with development of group program curriculum.
- Provide assertive outreach directed toward engaging and maintaining client relationships. Meet with clients individually when necessary or assigned.
- Participation in meal planning, shopping, and preparation as needed.
- Assist in offsite/group outings as directed by the APP Coordinator
- Provide assertive outreach directed toward engaging and maintaining client relationships in groups or 1:1.
- Provide interventions each day given group or individual needs.
- Contribute ideas and follow through with tasks as part of the treatment team.
- Maintain detail to timeliness for workday, scheduled meetings, supervision, DER submission for self and team.
- Self-directedness in prioritizing tasks, adapting quickly to changes in group setting.
EDUCATION AND LICENSURE: Bachelor's degree required. Degree in Social Work, Psychology, or human service field preferred. Equivalent work experience in the human services field, or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education, may be considered. EXPERIENCE: Required: 6 months of volunteer or paid experience working with individuals in the community. Preferred: experience working with individuals with mental health issues.
