Location:Lee Health -2776 Cleveland AveFort Myers FL 33901 Department: Care Management Work Type: Full Time Shift: Shift 1/ to Minimum to Midpoint Pay Rate:$31.78 - $42.90 / hour SummaryActively participates in assessment and identification of learning needs for acute care units, considering the patient population and the types of staff/roles employed in the departments of Care and Utilization Management. Demonstrates the ability to use unit and system data and audit tools to identify trends and patterns that indicate educational needs. Provides support as a change agent to help staff understand the rationale and desired outcome of change. Designs, develops, presents, coordinates appropriate learning solutions for identified needs, considering different learning styles of staff. Collaborates with directors, supervisors, care management staff and community agencies as needed on performance improvement activities and education projects. Effectively performs in the role of clinician, facilitator, consultant and/or researcher. Actively participates in the evaluation of work flow processes and patient safety issues and recommends effective, innovative solutions to improve patient care. Prioritizes department goals/objectives in accordance with patient and staff needs, unit and system goals. Requirements Educational Requirements
Basic Life Support mandatory by hiring date. Professional certification in a Care Management. OtherKnowledge of care and utilization management theories and practices, adult learning principles and methods related to instructional design and program evaluation. Demonstrated commitment to life-long learning. Active participation in leadership activities and change initiatives at the department level. Membership in professional/academic organizations desired.
