Are you ready to take the lead in transforming patient care across the continuum? Were looking for an experienced and dynamic leader to guide our Care Management team in delivering high-quality, cost-effective, and patient-centered care.

In this pivotal role, you will:

• Lead a multidisciplinary team of Case Managers and Social Workers, supporting their development through supervision, training, and mentorship.

• Drive clinical excellence and operational efficiency by ensuring appropriate, cost-effective care for our patients at every stage of their journey.

• Serve as a key liaison between the Care Management team and physicians, clinical departments, and support services to ensure seamless communication and collaboration.

• Collaborate with senior leadership including the Vice President, Directors, and Medical Director to oversee campus-specific fiscal and operational management of the department.

• Streamline workflows and optimize care transitions, ensuring patients receive the right care, at the right time, in the right setting.
