The Complex Care Navigator (Medicare Advantage) helps patients/families/caregivers facing serious illness and/or complex care needs, navigate the health care systems and collaborates with the Personalized Care Team (PCT) to incorporate patient goals with medical reality. The goal for the CCN is to form strong relationships with patients/families/caregivers, to bridge the gap between medical knowledge, the healthcare system, and the patient's lived experience and worldview. This is a non-clinical role with strengths in relationship building and communication, high emotional intelligence or "EQ" is required. This is high volume, customer service environment. You'll need to be efficient, productive and thoroughly dealing with our members over the phone. You also must be able to work on a flexible schedule that includes evening hours. Schedule: 4 10-hour shifts (7am-5:30pm PST) or 5 8-hour shifts (8am-4:30pm PST). No weekends are required. Location: Training will be onsite at 3901 Hoyt Ave. Everett, WA for about 4 weeks. Following training, the role will be hybrid (2 days onsite, 3 days remote), working from the following Marysville locations, depending on business need:
