- Community Health Worker Supervisor - Temporary Contract Position.
- The Loudoun County Health Department (LCHD) seeks an inspiring and organized Community Health Worker Supervisor to oversee outreach, education, and resource linkage efforts. This position plays a pivotal role in connecting underserved communities to vital resources, promoting healthier lifestyles, and making a lasting impact.
Role & Responsibilities
- Supervise the daily activities of Community Health Workers (CHWs) to ensure effective implementation of health and wellness programs.
- Manage the planning, coordination, and evaluation of staff activities, ensuring alignment with LCHD goals and community health needs.
- Support team members in the development of individualized work plans that contribute to the overall mission of public health.
- Monitor staff performance and project outcomes, applying quality improvement methods where necessary.
- Ensure compliance with all Loudoun County policies and guidelines in program activities.
- Serve as a liaison between the CHW team and the Population Health team, participating in collaborative efforts and reporting progress to the Population Health Manager.
- Communicate effectively with people outside the organization, representing LCHD to customers, the public, government agencies, and other external sources through in-person interactions, writing, telephone, or email.
- Develop and maintain constructive and cooperative working relationships with team members, supervisors, and external stakeholders over time.
- Provide information to supervisors, co-workers, and subordinates via phone, email, or in person, ensuring clear and effective communication.
- Adapt to evolving public health needs by taking on additional responsibilities as required to meet community health goals.
- Knowledge of Public Health Principles: Basic knowledge of public health practices, epidemiology, disease prevention, health promotion, and social service systems, including the roles of government agencies and community organizations in addressing health needs.
- Community Experience: Experience working with priority or underserved communities, with an understanding of relevant unique challenges and resources.
- Cultural Competency: Ability to communicate effectively with diverse populations, recognizing and respecting cultural norms, practices, and values within the community.
- Performs other duties as assigned.
Required Qualifications:
- Experience: At least two years in management and supervision, ideally in public health or a related field.
- Skills: Strong organizational, leadership, and problem-solving abilities.
- Technology: Proficiency in Microsoft Office and adaptability to new tools.
- Communication: Clear, concise, and effective in engaging diverse audiences.
Preferred Qualifications:
- Education: Bachelor's degree in health, human, or social sciences.
- Experience: Three years in health or social services, with familiarity in trauma-informed practices.
- Local Knowledge: In-depth understanding of Loudoun County's community resources.
Mandatory:
- Bilingual Spanish speaker to assist with the business needs.
- Proficiency in a second language (especially relevant languages in the community served) is highly preferred to effectively communicate with a broader population.
