About the Team

The Senior Services team at Lakewood Health System provides compassionate care across long-term care, assisted living, and home-based services. The team is dedicated to delivering exceptional, person-centered care while upholding the highest standards in safety, quality, and service. The Quality Coordinator works collaboratively with nursing, leadership, support staff, and external partners to foster a culture of continuous improvement and regulatory readiness.

Mission for Position

To lead and support quality improvement and infection prevention efforts across all senior services settings, ensuring consistent excellence in care and compliance with regulatory standards.

Duties & Responsibilities

* Ensure compliance with all applicable state and federal regulations related to long-term care, assisted living, and home-based services.

* Lead quality improvement initiatives using data-driven methodologies such as PDSA and LEAN to enhance outcomes and operational performance.

* Develop and oversee the Infection Prevention and Control Program (IPCP) across all senior care settings.

* Stay current on industry trends, regulations, and best practices in infection prevention and senior care.

* Partner with leaders to promote a culture of accountability, collaboration, and service excellence.

* Facilitate engagement with residents, tenants, and their representatives on quality and safety topics.

* Collaborate across departments to standardize best practices and drive innovation.

* Represent Senior Services in regulatory reviews and external forums.

* Manage data collection, audits, and documentation to support continuous readiness.

* Create and deliver staff education and training on infection prevention, quality standards, and regulatory expectations.

* Act as a subject matter expert for frontline staff and leadership regarding infection control and quality improvement practices.

Position Type



  • This is a full-time position (1.0 FTE)
  • Monday- Friday schedule with occasional evening or weekend hours, as needed on work assignments.

Qualifications

Qualifications

* A minimum of a two-year health-related associate degree (e.g., Nursing (ADN), LPN, Health Information Technology, Medical Assisting, OTA, PTA, Human Services, Healthcare Administration).

* Certification in Infection Control preferred or must be obtained within three years of hire.

* Minimum of three years of relevant healthcare, public health, or social services experience; at least one year in senior care services preferred.

* Demonstrated ability to lead and educate multidisciplinary teams.

* In-depth knowledge of infection control practices and regulatory requirements.

* Proactively leads quality initiatives and fosters a collaborative environment.

* Demonstrates attention to detail in infection prevention and compliance tracking.

* Builds strong partnerships across departments and disciplines.

* Communicates clearly and effectively with teams, residents, and external partners.

* Champions continuous improvement in all aspects of senior care.
