Providence Health Plan is calling a Clinical Care Coordinator who will:



  • Be responsible for coordinating multiple aspects of care for patients with preventative care needs, chronic diseases, and care opportunities within the Physician Enterprise
  • Conduct daily work that is inclusive of ordering laboratory tests or imaging exams per protocol to facilitate patient screenings, management of preventative care, and supporting optimization of chronic disease treatments as prioritized thru Enterprise Clinical governance
  • Assure that an integrated approach to the patients' care coordination is fulfilled
  • Serve as the liaison between Providence Clinical Network providers, primary care clinics, specialist offices, patients, multiple Providence Health scheduling and ancillary departments (e.g., Laboratory), and freestanding care delivery centers or services for population health programs and related processes
  • Perform all duties in a manner that promotes team concept and reflects the Sisters of Providence mission and philosophy.


We welcome 100% remote work for residents of Oregon who live within a daily commutable distance (20-25 miles) of our office location listed below:

Required Qualifications:



  • H.S. Diploma or GED
  • 1 year For certified Medical Assistant, a minimum of 3 years of medical assisting work experience in primary/specialty care
  • 1 year For Certified Medical Assistant, a minimum of 3 years of medical assisting work experience in primary/specialty care.


Preferred Qualifications:



  • Bachelor's Degree
  • Upon hire: Completion of an accredited Medical Assistant program and registered with State Board (where required by state) and/or certified through a nationally recognized Medical Assistant program such as AAMA, AMT, RMA, CCMA or NCCT Or
  • Upon hire: Nationally Certified Pharmacy Technician or registered with the State Board
