As a member of the Orthopedic Trauma team, the Patient Experience Rep I will work to support the administrative operations of the Orthopedic Trauma Program patient outreach, under the supervision of the Senior Program Coordinator of the Trauma Service. The Patient Experience Rep I will ensure timely follow-up care for patients with orthopedic injuries. The role involves coordinating outreach efforts to patients and families, scheduling follow-up appointments, and facilitating communication between patients, providers, and clinical teams to enhance care continuity and outcomes for patients with orthopedic trauma injuries.
Additionally, it includes reviewing and implementing care plans, referring patients to relevant services, and acting as a liaison with internal and community resources. Performs administrative tasks requiring basic knowledge of programs and services while providing positive and effective customer service that supports the Orthopedic Trauma Program, as well as departmental and hospital operations Principal Duties and Responsibilities:
