The Medical Social Worker is a licensed social work professional who is responsible for the delivery of clinical social work services to meet the medically related psychosocial needs of patients and families served by the hospital, in a manner reflective of the department's philosophy and objectives. Provides consultation to hospital and medical personnel related to biopsychosocial issues, continuity of care and discharge planning functions.

I. Major Responsibilities:

1. Assesses patient's need for post acute care through review of the medical record consultation with the Discharge Planning Team and dialogue with patient and/or designee.
2. Identifies community resources necessary to provide post discharge care for patient/and or designee.
3. As necessary, completes necessary regulatory paperwork required for admission to Skilled Nursing Facilities.
4. Collaborates with physicians, case managers, discharge planners, and other health care professionals to identify barriers to discharge or treatment.
5. Refers patients to Clinical Social Workers for counseling around issues of loss, anger, fear, bereavement, and depression.
6. Advocates for patients and families as they negotiate the health care system so that effective care, treatment, and planning may occur.
7. Documents Social Work intervention and/or other duties related to discharge plan.

II. Position Qualifications:

License/Certification/Education:

Required:
1. BSW or BS in Human Services.

Preferred:
1. Massachusetts Social Work License (LSW).

Experience/Skills:

Required:
1. Minimum of 1 year Human Services experience or relevant Human Services internship as approved by hiring manager.
2. Demonstrated ability in psychosocial assessment, knowledge of community resources, discharge planning ability, and ability to work with a multidisciplinary team.
3. Knowledge of biopsychosocial aspects in the Managed Care/Health Care field.
4. Excellent written and communications skills.
5. Computer literacy.

Preferred:
1. 3 years of Human Services experience.

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.
