We are pleased to present the following position for your consideration:

Overview: Heritage Behavioral Health Center is seeking a full time LPN to provide care to patients with mental health and behavioral health needs. This position will support the Crisis Unit by playing a critical role in the coordination, administration, and monitoring of medications, as well as supporting the overall health and wellness of patients in both the crisis unit and residential rehabilitation settings. This position will involve collaboration with pharmacy services, preparation and distribution of self-administered medications, and wellness checks for patients. The nurse will work closely with other healthcare providers to ensure comprehensive patient care.

This is a full-time position requiring a commitment to a rotating 12-hour schedule (6:30 AM - 6:30 PM), with every other weekend off. A $2/hour shift differential applies to all hours worked on Saturdays and Sundays.

Core Responsibilities Include:

  • Partner with Behavioral Health staff to support and monitor patients during self-administration of medications in the Crisis Unit and Residential Rehabilitation settings.
  • Collaborate with the pharmacy to ensure timely preparation, delivery, and availability of medications, including refills and dosage adjustments.
  • Coordinate referrals and linkages to appropriate medical services for patients requiring continued healthcare support.
  • Administer and follow up on 2-step TB testing for Residential Rehab clients, ensuring accurate documentation and compliance.
  • Lead weekly wellness groups for Residential Rehab clients every week, focusing on mental, physical, and emotional well-being.
  • Perform regular wellness checks, including monitoring of vital signs and general physical health, while noting concerns related to mental health or crisis recovery.
  • Document and promptly report any notable changes in patient condition or behavior.
  • Administer long-acting injectable medications per provider orders and protocols.
  • Maintain clear, timely communication with Crisis Unit staff regarding patient status, medical needs, and emerging concerns.
  • Support triage, assessment, and coordination of urgent healthcare needs in collaboration with medical and behavioral health teams.

Knowledge, Skills and Abilities:

  • Ability to engage individuals and decrease barriers to accessing treatment
  • Ability to solve problems and multi-task and coordinate a high level of activities.
  • Considerable interpersonal as well as oral and written communication skills
  • Ability to work in a team-based environment
  • Strong customer service skills
  • Ability to respond effectively in an emergency situation for patient care, possess strong listening and communication skills, and be attentive to detail.

Education and Experience:

  • Active undisciplined State of Illinois Licensed Practical Nurse (LPN) licensure.
  • Excellent organizational skills and attention to detail.
  • Ability to work independently and as part of a multidisciplinary team.
  • Compassionate and empathetic demeanor with a commitment to patient-centered care.

Salary Range: $29 - $34, depending on education.
