Requisition ID req31968 Working Title Program Specialist Position Grade 12 Position Summary The mission of the Department of Family and Community Medicine (FCM) is to improve the health and healthcare of the people of New Mexico and their communities through education, research, patient care, service, population health interventions and health policy development. FCM seeks to hire a Program Specialist to help manage the grant funding, contingency management program, and expansion of services for the UNM FCM Milagro program, which is focused on caring for pregnant and postpartum people with a substance use disorder. The Specialist for this funding/project will be encouraged to connect with patients as they transition from the hospital to the Milagro Clinic for services, engage and manage the Contingency Management program, promoting advertising for Milagro Program, and reach out to clinics and DOH offices around the state interested in partnering with Milagro. In addition to this there is need for data collection and reporting for grant funding outcomes. This position is full-time FTE and the salary will come from the Blue Cross Blue Shield funding for Milagro Program. Duties include: -Manage contingency management program for pregnant/postpartum people with stimulant use disorder -Connect with patients as they transition from hospital discharge to outpatient follow-up. -Collect and manage data. -Prepare monthly reports for funding -Support advertising and telehealth expansion for Milagro Program -Identify barriers to care for patients and help support their needs through program funding. * * See the Position Description for additional information. Minimum Qualifications High school diploma or GED; at least 6 years of experience directly related to the duties and responsibilities specified. Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis. Preferred Qualifications Preferred Qualifications: * Experience working with Power Chart, Microsoft Outlook, Word, Excel, and PowerPoint. * Experience as Peer Support Worker/Specialist, Community Health Worker, or lived experience with substance use * Excellent customer service skills and problem-solving skills.
