Summary:
The SW Supervisor of Care Management is responsible for supervision of assigned staff. Assesses, plans, implements and evaluates delivery of psycho-social patient care in accordance with departmental standards of care and practice. Supports the goals and objectives of the organization and department in collaboration with the Nurse Manager and Director. Participates in activities that promote professional development and enhance the practice of care management.
Responsibilities:
- Provides leadership and direction within SW scope of practice in accordance with organizational and department goals and objectives.
- Recruits and onboards new staff, in an effort to build diverse work teams. Supervises employees, provides ongoing communication and monitoring of job expectations and competencies. Contributes to evaluation of staff and provides coaching, when appropriate.
- Assists in planning, maintaining, and communicating the staffing plan for unit and scheduling staff based on staffing plan. Coordinates and delegates appropriate staff to deliver patient care in a calm, efficient manner.
- Applies advanced clinical professional social work skills to professional social work practice with individuals, families, and groups. Collaborates with the interdisciplinary team to coordinate care management services. Provides direct care management to an identified group of patients. Serves as a clinical resource person.
- Communicates with kindness and respect. Encourages teamwork among members to achieve a common goal. Demonstrates professionalism with team members, patients and families.
- Practices in compliance with applicable laws, regulatory agencies (i.e., The Joint Commission, Counselor and Social Worker Board of Ohio, National Committee for Quality Assurance) and other governing bodies. Assists with coordination of department performance improvement activities to meet department metrics and program requirements.

Other information: Technical Expertise
- Experience with healthcare coordination in a managed care environment, homecare or community agency is preferred. Experience working in a parent-child exposure setting is required.
- Critical and analytical reasoning, astute clinical judgement, strong communication (verbal and written) interpersonal, organizational, and leaderships skills required.
- Demonstrates professional, appropriate, effective, and tactful communication skills, including written, verbal, and non-verbal.
- Proficiency in MS Office [Outlook, Excel, Word] or similar software is required. Epic software or similar EMR software is preferred.
Education and Experience
- Master of Science in Social Work degree with a current unrestricted license as an Independent Social Work Supervisor (LISW-S) in Ohio.
- A minimum of 3 years of relevant pediatric social work is required. A minimum of 2 years in case management and/or care coordination is preferred. Leadership experience is preferred.
- Certification by a nationally recognized care management organization preferred.
