Position Summary

The
primary goal of the Senior Employee Participant Health Equity and Social
Connectedness for Older Adults Program (SEP HE+) is to promote healthy aging in
New Mexico by developing and implementing Social Drivers of Health (SDOH)
interventions and increasing access to resources for improved health equity and
wellbeing of adults age 55+ in Bernalillo/Sandoval Counties.

Partner
Health Extension Regional Offices (HEROs) address SDOH needs by identifying
community health priorities, collaborating to address systems issues,
connecting to resources, and strengthening resource networks. Under limited
supervision, the HE+ Partner HERO will plan and implement initiatives in collaboration
with community partners, designed to improve the health and health equity for
Older Adults in the state of New Mexico. The HE+ Partner HERO will collaborate with the CHW/R(s) working on this project
in Bernalillo/Sandoval Counties to address unmet resource needs such as
volunteer income tax assistance (VITA) sites, public transportation, social
connection and cohesion, remote/in-home health monitoring, and interventions.

Conditions of Employment
  • Employees who provide services or work in patient care or clinical areas are required to be in compliance with the University's influenza vaccination requirement.
  • If located at the UNM Children's Campus, successful candidate must pass a comprehensive post-offer, pre-employment criminal and New Mexico Children, Youth and Families Department (CYFD) fingerprint background check, and a continued successful status will be verified by periodic future background checks.


Minimum Qualifications

High school diploma or GED; at least 6 years of experience directly related to the duties and responsibilities specified.

Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.



Preferred Qualifications
  • Plans, develops, implements and coordinates the SEP HE+ Partner Health Extension Rural Offices (SEP HE+ Partner HEROs) program in Bernalillo/Sandoval Counties for Older Adults age 55+
  • Performs community liaison and outreach activities for SEP HE+ program, acting as a point of contact between communities and the HSC
  • Working with communities, identifies primary health issues within the aging population and helps provide and promote timely programs and interventions, linking with HSC resources as necessary
  • Collaborates with local Community Health Workers to address the needs of Older Adults
