Coordinated Entry Housing Navigator - Housing & Homeless Services

Community Teamwork is seeking a Coordinated Entry Housing Navigator who will be responsible for overseeing the Coordinated Entry Process for the North East region of the Balance of State. This includes reviewing and assessing the Coordinated Entry (CE) Vulnerability Tool to document and track the most vulnerable participants in the region and coordinate a regional response. This position ensures compliance with program goals and regulations as well as the reporting responsibilities to the Department of Housing and Community Development.

Responsibilities include:



  • Direct the administration and management of the Coordinated Entry process in the North East region of the Balance of State's Continuum of Care
  • Collecting and reviewing assessments to identify most vulnerable participants
  • Develop and maintain tracking systems to coordinate regional efforts to support the rehousing of participants
  • Develop and maintain systems to increase communication and understanding of a coordinated response across all partnering agencies
  • Assist providers with improved access to services for homeless individuals
  • Develop and facilitate Coordinated Entry training



Qualifications include:



  • Master's Degree in a relevant field preferred. Bachelor's degree in relevant field preferred
  • Must have a minimum of four years of related work experience
  • 2+ years of program management experience including supervision of staff. Ability to maintain confidentiality within the workplace
  • Strong proven decision-making abilities


  • Proficient computer skills required in basic computer software and data collection systems
  • Excellent organizational, interpersonal, oral and written communication skills
  • Sensitivity to the needs of the participant population
  • Bilingual skills (English/Spanish or English/Khmer) a plus
