Job Summary The Medical Social Worker IV performs comprehensive psychosocial assessments and formulates therapeutic plans for patients/families affected by chronic kidney disease who receive services from renal /dialysis clinics throughout Cook County Health (CCH). The Medical Social Worker IV assists in developing a care plan to meet patients' needs by participating in patient/family and staff meetings and in-services that are directed at supporting and maximizing the social and psychological functioning and adjustment of a renal/dialysis patient. The Medical Social Worker IV collaborates with a multidisciplinary team for provision of comprehensive social work services and documents those services according to facility policies that meet Medicare and Quality Assurance and Performance Improvement (QAPI) requirements.
